PRIZE: First National Bank of Creston ("FNB") will give away one (1) $100 Anytime Access MasterCard Prepaid Card (the "Prize") during the Promotion period (defined below) of its "Make the Switch Gift Card Give away" eStatement Promotion (the "Promotion") in accordance with these Official Rules. Total Maximum Value of All prizes awarded during the promotion period: $100.00.
ENTRY PERIOD: The promotion begins on August 1, 2012 and ends on October 31, 2012 at 5:00 pm (the "Promotion Period").
HOW TO ENTER: Subject to the "Conditions of Entry," below, there are two (2) ways to enter. You can enter this Promotion online or by regular mail.
Automatic entry: Enroll online or at a bank location for eStatement delivery from First National Bank. To enroll in eStatement delivery, login to your First National Bank checking account at www.fnbcreston.com ("website" and select the accounts you want enrolled in eStatements.
- To enter without purchase: On a plain 3" x 5" card, legibly handprint or type your name, complete street address, city, state, zip code, home telephone number, and email address and mail the card to: First National Bank eStatement Promotion, 101 W. Adams St., Creston, IA 50801. Mail entries must be postmarked and received by October 31, 2012. One entry per envelope is permitted.
CONDITIONS OF ENTRY: No purchase is necessary to enter this promotion. This promotion is open to legal residents of the fifty (50) United States and the District of Columbia, 18 years of age and older as of September 1, 2012. Void in Puerto Rico and where prohibited. Employees, directors and officers, and their immediate families (spouse, parents, children, siblings, and their respective spouses, regardless of where they reside), and their respective household members, whether or not related, of The First National Bank in Creston are not eligible to enter. The drawing is subject to all applicable federal, state, and local laws and regulations.
ODDS OF WINNING: Odds of winning will vary depending on the number of eligible entries received.
DRAWING: Subject to these Official Rules, a designated First National Bank official will select the Prize winner by random drawing from among all eligible entrants on or around November 5, 2012. The prize winner will be notified by email, phone, or regular mail. The prize winner must claim his or her prize within ten (10) business days after the date of First National Bank's first notification attempt, which shall be the date notification is sent if by email or regular mail, and the date the call was placed to the winner, if by phone. The prize winner must claim the Prize at First National Bank located at 101 W. Adams St., Creston, IA 50801. If the prize winner is unable to come to that location to claim the Prize, he or she may call Sarah Young at First National Bank at 877.782.2195 to make alternate arrangements, if available, to claim the prize.
If a potential Prize winner fails to claim the Prize within the ten (10) business day period of time, is found to be ineligible, or fails to comply with these Official Rules, or any of the requirements, the potential Prize winner will be disqualified and an alternate potential Prize winner, randomly selected from the remaining eligible entries, at First National Bank's sole discretion, will be contacted. Decisions with respect to the Prize winner and other matters pertaining to this Promotion shall be final.
REQUIREMENTS OF THE PRIZE WINNER: The Prize winner will be required to sign a Media Release as part of determining the Prize winner's eligibility and, prior to, and as a condition of, his or her acceptance and use of the Prize.
PRIZE RESTRICTIONS: Any expenses not specified relating to the acceptance and use of the prize are the responsibility of the winner.
LIMITATION OF LIABILITY: First National Bank is not responsible for printing or typographical errors in any related materials, or for stolen, lost, late, misdirected, damaged, incomplete, illegible, or postage-due mail or entries. First National Bank reserves the right to cancel or modify the drawing if fraud or technical failures compromise the integrity of the drawing as determined by First National Bank in its sole discretion. This drawing is governed by the laws of the United States.
